Job Description
Advance your career in home health as a Nurse Practitioner, conducting risk assessments for Medicare Advantage plan members across Grant County, INincluding Marion, Gas City, and Upland. This per diem opportunity provides the flexibility to design your own schedule, with shifts available any day between Monday and Sunday. Youll play an essential role by evaluating patients in their homes, supporting proactive health management, and making a positive impact in the community.
Schedule:
- Flexible shifts available between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage health plan members.
- Review and evaluate patients prior and current medical history, chronic conditions, and medications.
- Deliver patient education regarding health conditions and medication management.
- Identify gaps in care and provide recommendations for improving health outcomes.
- Note: No treatment plan creation or prescribing of medications required.
Qualifications:
- Valid Nurse Practitioner license in Indiana (IN)
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Minimum of 6 months of recent NP experience preferred
- MSN degree from an accredited school of nursing
This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and administrative time pay.
