Make an impact delivering high-quality care in a per diem role as a Nurse Practitioner specializing in home health risk assessments. Based in the Virginia Beach area, you will travel to patients residences within Norfolk, providing invaluable assessments for Medicare Advantage plan members. This unique position offers significant flexibility and autonomy, ideal for those who excel working independently while making a difference in patient lives.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule to suit your availability
• Choose from minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct comprehensive annual risk assessments for Medicare Advantage patients in their homes
• Review and evaluate current and prior medical history, health conditions, and medication use
• Offer education and guidance on medical conditions and medication management
• Identify care gaps and make evidence-based recommendations to enhance patient outcomes
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license in Virginia
• Active board certification through AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred
• CPR certification (BLS)

This position offers $80$90 per evaluation, plus a mileage stipend and compensation for administrative time, reflecting the importance and flexibility of this role.
