Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule to suit your availability
- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments with an average evaluation time of 45 minutes each
- Review and evaluate patients past and present medical history, conditions, and medication records
- Offer education to patients regarding chronic conditions and medications, helping to empower their health choices
- Identify care gaps and recommend strategies for their improvement
- No need to create treatment plans or prescribe medications
Qualifications:
- Valid Nurse Practitioner license in the state of VA
- Current CPR (BLS) certification
- Active Board Certification via AANP or ANCC
- At least six months of current NP experience preferred
- Graduate of an accredited MSN program
