Job Description

A per diem opportunity is available for a skilled Nurse Practitioner specializing in home health risk assessments. This role serves Medicare Advantage plan members throughout Craig County, including Vinita, Ketchum, Welch, and White Oak. The position involves traveling within these areas to conduct thorough in-home evaluationseach lasting approximately 45 minutes and including either an annual health assessment or a post-discharge evaluation.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling: create your own work calendar
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members in the Craig County area
  • Review medical history, current health conditions, and medication use with patients
  • Educate patients on their medical conditions and medications
  • Identify any gaps in care and offer actionable recommendations
  • Note: There is no responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Licensed Nurse Practitioner in Oklahoma
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of six months recent experience as an NP preferred
  • MSN degree from an accredited nursing school
