Embark on a meaningful per diem opportunity delivering care directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. Work within St. Landry County, traveling to areas such as Opelousas, Eunice, and Port Barre. This unique role focuses on Medicare Advantage plan members, providing an essential service that helps maintain and enhance their well-being.
Schedule:
- Flexible shifts available seven days a week
- Ability to customize your own schedule
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Complete comprehensive, annual home health risk assessments in patients homes (each assessment typically takes about 45 minutes)
- Review prior and current medical history, evaluate medical conditions, and assess medication management
- Educate patients and caregivers about ongoing conditions and medication usage
- Recognize and document any gaps in care, providing actionable recommendations for further attention
- Focus on assessmentsno need for establishing treatment plans or writing prescriptions
Qualifications:
- Active Nurse Practitioner license in the State of Louisiana
- Valid CPR (BLS) certification
- Board certification (AANP or ANCC)
- MSN degree from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
In this per diem position, youll earn $80$90 per evaluation, along with a mileage stipend and compensated administrative time.
