Step into a rewarding per diem Nurse Practitioner role focused on community care and flexible scheduling. Utilizing your expertise in home health, you will conduct comprehensive risk assessments for Medicare Advantage members living across Avery County, including areas such as Banner Elk, Beech Mountain, Crossnore, and Elk Park.
Schedule:
- Flexible shifts available between Monday and Sunday
- The ability to design your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments during home visits
- Evaluate prior/current medical history, chronic conditions, and medication usage
- Deliver patient education on medical conditions and medications
- Identify gaps in care, offering recommendations for enhanced outcomes
- Focus on assessment and educationno treatment plans or prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board Certification (AANP or ANCC)
- At least 6 months of recent NP experience preferred
- MSN from an accredited school of nursing
