Take your advanced practice skills out of the clinic and into the community, providing comprehensive home health risk assessments for Medicare Advantage members. This per diem opportunity offers the autonomy to create your own schedule with shifts available seven days a week, and the flexibility to tailor your weekly or bi-weekly commitment according to your preferences. Assignments are located throughout Victoria County, ensuring a dynamic and engaging workday.

Key Responsibilities
- Perform annual health risk assessments in members homes throughout Victoria County
- Review medical history, current conditions, and medication usage
- Deliver education to clients and families about medical conditions and medications
- Pinpoint and communicate care gaps, recommending follow-up interventions as needed
- Focus on assessmentno prescribing or treatment planning involved
Qualifications
- Active Nurse Practitioner license in TX
- Board certification through AANP or ANCC
- Minimum of 6 months current NP experience preferred
- Graduate degree (MSN) from an accredited institution
- Current BLS/CPR certification
In addition to clinical fulfillment, youll receive industry-competitive compensation of $80-$90 per evaluation, supplemented by a mileage stipend and compensation for administrative time.
