Experience the flexibility and autonomy of providing care as a Nurse Practitioner specializing in home health risk assessments throughout Fremont County, CO. This per diem opportunity allows you to conduct comprehensive assessments for Medicare Advantage plan members in their homes, tailoring your schedule to best fit your availability.
Schedule:

- Minimum weekly or bi-weekly hourly commitment to ensure work-life balance
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members in private residences within Fremont County
- Review and document medical history, existing conditions, and medication regimens
- Deliver education to patients regarding their medical conditions and medications
- Identify gaps in care and recommend actionable solutions
- No responsibility for creating long-term care plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Colorado
- Current BLS (CPR) certification required
- Board Certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
Compensation and Perks: This role offers $80$90 per completed evaluation, along with a mileage stipend for travel between patient homes and reimbursement for administrative time.
