Job Description

Make a significant impact by utilizing your clinical expertise in a dynamic per diem role focused on home health risk assessments. As a Nurse Practitioner, you will provide essential evaluations for Medicare Advantage plan members throughout Union and the greater Broome County area, including neighborhoods such as Binghamton, Vestal, and Johnson City.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to accommodate personal commitments
  • Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior medical history, current health status, and medication usage
  • Educate members about their health conditions and medication regimens
  • Identify care gaps and recommend possible resolutions to close them
  • Focus exclusively on evaluationno requirement for treatment plans or medication prescription

Qualifications:

  • Active Nurse Practitioner license in New York State
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • Minimum six months of current Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program

In this position, youll be compensated for each evaluation performed, ranging from $80 to $90 per assessment, in addition to a mileage stipend and administrative time. This structure allows you to control your earning potential while maintaining a flexible scheduleideal for a provider seeking autonomy and variety in their work.
