Job Description

Experienced Nurse Practitioners are needed to complete home health risk assessments with a focus on providing high-quality care to Medicare Advantage plan members. This per diem opportunity allows you to make a meaningful impact within Shelby County, Ohio, including visits throughout Sidney and surrounding communities such as Anna, Fort Loramie, and Russia. You will independently visit members in their homes, performing thorough health evaluations averaging 45 minutes each. The assessments may include annual health check-ups or post-discharge follow-up visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule for optimal work-life balance
  • Minimum weekly or bi-weekly hour commitment tailored to your availability

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medications
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and make clinical recommendations to address these needs
  • Note: No prescribing of medications or development of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • At least 6 months of NP experience preferred
  • MSN degree from an accredited nursing program

Compensation and Benefits:

  • Earnings range from $80$90 per evaluation
  • Mileage stipend provided
  • Compensation for administrative time associated with assessments
