Job Description

Exciting per diem opportunity for experienced Nurse Practitioners who are passionate about providing patient-centered care in a home health setting. This position focuses on conducting risk assessments for Medicare Advantage plan members residing in Sidney and surrounding communities within Shelby County, including Anna, Fort Loramie, and Russia.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own work schedule for optimal work/life balance
  • Commit to as many or as few hours each week or bi-weekly as you prefer

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments in patients homes (approximately 45 minutes per visit)
  • Carefully review medical, surgical, and medication histories for each patient
  • Provide clear education regarding diagnoses and medication regimens
  • Assess care gaps and offer recommendations for improved outcomes
  • No requirements for developing treatment plans or prescribing medications

Qualifications:

  • Active and unrestricted Nurse Practitioner license in Ohio
  • Board-certified by AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited institution
  • Minimum of 6 months recent experience as a Nurse Practitioner preferred

This per diem role offers $80-$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
