Deliver vital healthcare to your community by providing in-home risk assessments as a skilled Nurse Practitioner. This per diem opportunity focuses on flexibility and meaningful patient engagement across Dane County, assessing Medicare Advantage members in settings like Madison, Sun Prairie, and Fitchburg.

Schedule:

• Flexible shifts available from Monday to Sunday
• Design your own work schedule
• Commit to a minimal weekly or bi-weekly hour requirement

What Youll Do:

• Complete comprehensive home health risk assessments for Medicare Advantage plan members
• Conduct detailed evaluations of patient medical history, current conditions, and medication usage
• Provide patient education regarding medical conditions and prescribed medications
• Identify care gaps and recommend solutions, ensuring improved patient health outcomes
• Focus solely on assessmentno treatment plan creation or medication prescribing

What We Look For:

• Active Nurse Practitioner license in Wisconsin
• Current BLS (CPR) certification
• Board certified through AANP or ANCC
• Minimum of 6 months current NP experience (preferred)
• Master's degree in Nursing from an accredited program
