This per diem opportunity involves conducting home health risk assessments for Medicare Advantage plan members in Windham County, CTincluding Windham (Willimantic), Killingly, and Mansfield. Each assessment typically takes about 45 minutes and may be an annual health evaluation or a post-discharge visit. Enjoy the flexibility to create your own schedule, working as much or as little as you prefer, with minimum weekly or bi-weekly hour commitment options.

Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members.
- Review and evaluate both past and present medical histories, conditions, and medication use.
- Educate patients about their health conditions and medications to promote understanding and compliance.
- Identify gaps in care and collaborate to recommend solutions.
- No requirement to prescribe medications or initiate new treatment plans.
Qualifications:
- Active Nurse Practitioner license in Connecticut.
- Current CPR certification (BLS required).
- Board Certification via AANP or ANCC.
- Minimum of 6 months current NP experience preferred.
- MSN from an accredited nursing school.
