Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments across Erie County, including Sandusky, Huron, and Vermilion, OH. This flexible role provides variety and autonomy while making a direct impact on the well-being of Medicare Advantage plan members.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Set your own schedule
- Meet minimum weekly or bi-weekly hourly commitments as fits your availability
Key Responsibilities:
- Perform in-home annual risk assessments for Medicare Advantage plan members
- Review prior and current medical history, conditions, and medication use with patients
- Educate patients on medical conditions and medication regimens
- Identify care gaps and offer recommendations for improved health outcomes
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Current Nurse Practitioner License in Ohio
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of recent experience as an NP preferred
