Job Description

Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments across Erie County, including Sandusky, Huron, and Vermilion, OH. This flexible role provides variety and autonomy while making a direct impact on the well-being of Medicare Advantage plan members.

Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Set your own schedule
  • Meet minimum weekly or bi-weekly hourly commitments as fits your availability

Key Responsibilities:

  • Perform in-home annual risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, conditions, and medication use with patients
  • Educate patients on medical conditions and medication regimens
  • Identify care gaps and offer recommendations for improved health outcomes
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Current Nurse Practitioner License in Ohio
  • Active CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least 6 months of recent experience as an NP preferred
