Lead home health risk assessments for Medicare Advantage members in Spokane County, WA, as a Nurse Practitioner specializing in home visits. This highly autonomous per diem position offers the ability to create your own schedule and aims to provide proactive, patient-centered care within members homes. Each home health visit, typically lasting about 45 minutes, can include annual assessments or post-discharge check-ins at various locations throughout Spokane County, including Spokane Valley and Opportunity.

Schedule:

• Flexible shifts available Monday through Sunday
• Customize your weekly or bi-weekly hours
• Choose shifts that best fit your availability

Key Responsibilities:

• Complete home health risk assessments for Medicare Advantage plan members
• Review and document prior/current medical histories, chronic conditions, and medication use
• Provide education to patients regarding health conditions and medication management
• Detect care gaps and offer recommendations to support overall health
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Unrestricted Nurse Practitioner license in Washington
• Current BLS/CPR certification
• Board Certification through AANP or ANCC required
• Master of Science in Nursing (MSN) from an accredited program
• At least 6 months of recent experience as an NP is preferred

Compensation for this opportunity is $80$90 per completed evaluation, along with a mileage stipend and payment for administrative time.
