Opportunity for a skilled Nurse Practitioner to provide impactful care in a flexible home health setting. This per diem position allows you to focus on patient-centered risk assessments for Medicare Advantage plan members throughout Craven County. Youll have the autonomy to create your own schedule and make a meaningful differencewithout prescribing medications or managing ongoing treatment plans.
Schedule:
- Flexible shifts available Monday through Sunday
- Design a schedule that works for you
- Commit to a minimum weekly or bi-weekly set of hours
Key Responsibilities:
- Perform thorough annual home health risk assessments in patients homes
- Review prior and current medical history, chronic conditions, and medication use
- Educate patients about their health conditions and prescribed treatments
- Identify any gaps in care, providing recommendations for follow-up
- Visits typically take 45 minutes, with variations including annual assessments or post-discharge check-ins
Desired Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification via AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing school
- At least 6 months of relevant NP experience preferred
