Job Description

Deliver high-quality care to patients in their homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity is designed for those wanting flexibility and autonomy while making a significant impact on Medicare Advantage plan members across Craven County, NC. Providers will conduct comprehensive risk assessments in locations such as Vanceboro, New Bern, Havelock, and Bridgeton. Each visit, lasting approximately 45 minutes, may include annual assessments or post-discharge evaluations, with mileage and administrative time compensated.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own schedule
  • Set your preferred minimum weekly or bi-weekly hours

Key Responsibilities:

  • Perform thorough home health risk assessments for Medicare Advantage members
  • Review prior and current medical and medication history
  • Educate members on medical conditions, treatment regimens, and medication management
  • Identify care gaps and recommend appropriate interventions
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months recent experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program

Compensation for this role is $80-$90 per evaluation, with additional reimbursement for mileage and administrative time.
