This per diem role offers unparalleled flexibility, allowing you to choose shifts and locations such as Gary, Hammond, and Merrillville. Each assessment lasts about 45 minutes, enabling a focus on quality care during annual health evaluations or post-discharge visits.

Schedule:

• Flexible shifts offered Monday through Sunday
• Self-scheduling: create your own workweek
• Minimum hourly commitment per week or bi-weekly

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate prior and current medical history, existing medical conditions, and medication histories
• Offer patient education regarding health conditions and medications
• Detect gaps in care and recommend solutions or interventions
• No requirements for treatment plan creation or medication prescribing

Preferred Qualifications:

• Active Nurse Practitioner license in Indiana
• Current Basic Life Support (BLS/CPR) certification
• Board Certified through AANP or ANCC
• At least 6 months recent NP experience is ideal
• Master of Science in Nursing (MSN) from an accredited institution
