Job Description

As a Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessments for Medicare Advantage members in a range of home settings throughout Lake County, including Gary, Hammond, Merrillville, and surrounding areas. Enjoy the autonomy of creating your own schedule, with shifts available any day of the week to fit your lifestyle.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct thorough annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Evaluate patients prior and current medical history, medical conditions, and medication usage
  • Educate patients regarding their conditions, medication regimens, and preventive care
  • Identify care gaps and make recommendations to address them
  • No prescribing or development of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in the state of Indiana
  • Master of Science in Nursing (MSN) degree from an accredited program
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • At least 6 months of current NP experience preferred

Youll be compensated $80-$90 for each evaluation, and will receive a mileage stipend along with compensation for necessary administrative time. This position allows you to focus on patient assessment and education, free from ongoing case management or complex treatment planning.
