Job Description

Experienced Nurse Practitioners are invited to deliver critical home health risk assessments to Medicare Advantage members throughout Utica and greater Oneida County. This per diem opportunity allows clinicians to make a significant impact by visiting individuals in their homesassessing health needs and supporting optimal outcomeswhile enjoying the flexibility to create their own schedules. The average home visit lasts 45 minutes and may range from annual health assessments to post-discharge follow-ups. Candidates may be scheduled in locations such as Rome, Whitesboro, New York Mills, or other areas within Oneida County.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete comprehensive, annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate patients prior and current medical and medication histories
  • Provide clear education to patients on medical conditions and medications
  • Identify care gaps and recommend next steps for follow-up care
  • No responsibility for direct treatment planning or medication prescribing

Qualifications:

  • Current Nurse Practitioner License in New York
  • Active BLS/CPR certification
  • Board Certification via AANP or ANCC
  • Preferably at least 6 months of recent Nurse Practitioner experience
  • MSN degree from an accredited nursing program
