Job Description

Deliver valuable care directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity offers the flexibility to create your own schedule, with shifts available any day of the week, Monday through Sunday. Assignments are within Oneida County, with potential visits in Utica, Rome, Whitesboro, and New York Mills. Candidates can expect a minimum weekly or bi-weekly hourly commitment and a supportive environment designed for meaningful patient interactions.

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members in various settings, including annual evaluations and post-discharge visits
  • Carefully review and assess prior and current medical history, health conditions, and medication use
  • Offer clear patient education regarding health conditions, wellness strategies, and medication adherence
  • Identify potential gaps in care, share findings, and provide informed recommendations to help patients and their care teams close those gaps
  • No requirement to generate treatment plans or prescribe medications

Qualifications:

  • Current Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • Active BLS (CPR) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred

This role features a competitive pay structure, offering $80$90 per evaluation, with a mileage stipend and compensation for administrative time.
