Job Description

Advance your clinical expertise and enjoy flexible scheduling as a Nurse Practitioner specializing in Home Health Risk Assessments. In this per diem role, youll provide exceptional patient-centered healthcare to Medicare Advantage plan members throughout Uniontown and surrounding areas in Fayette County, including Connellsville and Masontown. This position allows you to set your own schedule with shifts available any day of the week.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Design your own work schedule
  • Commit to minimum weekly or bi-weekly shift requirements

Primary Responsibilities:

  • Perform comprehensive annual home health risk assessments with visits averaging 45 minutes each
  • Review and evaluate medical history, current health status, and ongoing conditions
  • Deliver education to patients regarding medical diagnoses, medication use, and health management
  • Identify care gaps and make appropriate recommendations to support optimal health outcomes
  • Focus exclusively on assessmentno treatment planning or prescribing required

Desired Qualifications:

  • Active Nurse Practitioner (NP) license in Pennsylvania
  • Board certified through AANP or ANCC
  • MSN from an accredited nursing school
  • Current CPR certification (BLS)
  • Minimum of 6 months of NP experience preferred
