Schedule:

  • Flexible shifts, Monday through Sunday
  • Build your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments in patients homes
  • Evaluate and document medical and medication histories, as well as current health conditions
  • Educate patients regarding their medical conditions and medications
  • Identify gaps in care and offer recommendations to improve health outcomes
  • Assessments may range from routine annual visits to post-discharge evaluations
  • No prescribing or treatment plan development required

Qualifications:

  • Nurse Practitioner license, valid in Indiana
  • Board certification via AANP or ANCC required
  • MSN degree from an accredited nursing program
  • Active BLS (CPR) certification
  • At least 6 months of recent nurse practitioner experience preferred
