This per diem opportunity offers ultimate flexibility: create your own schedule with shifts available any day of the week and the freedom to set your weekly or bi-weekly commitment.
Schedule:
- Flexible shifts, Monday through Sunday
- Build your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patients homes
- Evaluate and document medical and medication histories, as well as current health conditions
- Educate patients regarding their medical conditions and medications
- Identify gaps in care and offer recommendations to improve health outcomes
- Assessments may range from routine annual visits to post-discharge evaluations
- No prescribing or treatment plan development required
Qualifications:
- Nurse Practitioner license, valid in Indiana
- Board certification via AANP or ANCC required
- MSN degree from an accredited nursing program
- Active BLS (CPR) certification
- At least 6 months of recent nurse practitioner experience preferred
