Job Description

Enjoy the autonomy and flexibility of per diem work as you bring your expertise as a Nurse Practitioner to home health risk assessments within Muskegon County, MI. This impactful role allows you to provide clinical insights for Medicare Advantage plan members in a variety of community settings, including annual health assessments and post-discharge visits. Each evaluation typically lasts 45 minutes, giving you the opportunity to deliver high-quality care and education without the pressure of developing treatment plans or prescribing medications.

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Create your own schedule based on your availability
  • Satisfy only a minimal weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform annual home health risk assessments on Medicare Advantage plan members
  • Assess both prior and current medical history, including chronic conditions and medication use
  • Offer patient education regarding health conditions and prescribed medications
  • Identify gaps in care, providing clinical recommendations for follow-up
  • Complete required documentation during or following each visit

Qualifications:

  • Active Nurse Practitioner license in Michigan
  • Current BLS/CPR certification
  • Board certified by AANP or ANCC
  • Minimum of six months' experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited program
