A unique opportunity awaits for licensed Nurse Practitioners to perform comprehensive home health risk assessments throughout Muskegon County, MI. This per diem position involves visiting Medicare Advantage plan members in their homes across cities such as Muskegon, Norton Shores, and Muskegon Heights. Each visit typically lasts about 45 minutes and may include annual health assessments or post-discharge check-ins, helping members better understand their conditions and connect with the care they need.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-directed scheduling; create your own hours
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, conditions, and medications
- Educate members on their medical conditions, prescribed treatments, and general health
- Identify gaps in care and provide recommendations for follow-up or additional support
- No direct treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Michigan
- Board certification (AANP or ANCC)
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months Nurse Practitioner experience preferred
