Job Description

Advance your clinical expertise in a flexible, per diem Home Health role providing dedicated care across Jerome County. This position focuses on conducting in-person risk assessments for Medicare Advantage plan members in the comfort of their own homes. Each visit averages approximately 45 minutes and may consist of routine annual evaluations or post-discharge assessments.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Customize your schedule to best fit your availability
  • Commitment to a minimum number of hours on a weekly or bi-weekly basis

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current conditions, and medication regimens
  • Educate patients about their health concerns and medication management
  • Identify care gaps and offer actionable recommendations to close them
  • No prescribing or creation of treatment plans required

Qualifications:

  • Active and unrestricted Nurse Practitioner license in Idaho
  • Current CPR certification (BLS)
  • Board Certification from AANP or ANCC
  • Minimum of 6 months recent nurse practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
