Job Description

Deliver impactful patient care in a dynamic per diem opportunity performing home health risk assessments throughout Tulsa County, OK. As a Nurse Practitioner in this role, you will visit Medicare Advantage plan members in their homesincluding locations such as Owasso, Broken Arrow, and Bixby. Each assessment takes approximately 45 minutes and could include either annual health reviews or post-discharge visits.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to meet your availability
  • Choose your preferred minimum weekly or bi-weekly commitment

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review patients medical histories, ongoing medical conditions, and medication usage
  • Deliver patient education regarding health conditions and prescribed medications
  • Identify gaps in care and offer actionable recommendations for follow-up
  • No treatment plans or prescribing requiredfocus on evaluation and education

Qualifications:

  • Current Nurse Practitioner license in Oklahoma
  • Active Board Certification through AANP or ANCC
  • Current CPR/BLS certification
  • Minimum of six months of current NP experience preferred
  • MSN degree from an accredited nursing program
