Bring your advanced practice expertise to a dynamic per diem position conducting home health risk assessments in Bedford County, TNincluding the areas of Shelbyville, Wartrace, and Bell Buckle. As a Nurse Practitioner specializing in home health, youll play a vital role in evaluating the needs of Medicare Advantage plan members during annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts offered Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual risk assessments in members homes, with each visit averaging approximately 45 minutes
- Review and evaluate medical histories, ongoing conditions, and medication regimens
- Offer education to patients regarding their health conditions and prescribed medications
- Detect gaps in care and provide actionable recommendations to help resolve them
- No requirement for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
Successful candidates will receive $80$90 per completed evaluation, including a stipend for mileage and administrative time.
