Role Overview You will visit Medicare Advantage members in their homes across various communities in Franklin County (including Winchester, Decherd, and Estill Springs) to conduct health risk assessments. Most visits last about 45 minutes, focusing on annual health evaluations or post-discharge needs. The primary responsibilities include:

Key Responsibilities

  • Conduct comprehensive annual home health risk assessments
  • Review and evaluate patients prior and current medical history, existing conditions, and medications
  • Educate patients on their health conditions and prescribed medications
  • Identify care gaps and provide actionable recommendations to improve health outcomes
  • No requirement to develop treatment plans or prescribe medications

Qualifications

  • Active Nurse Practitioner License in TN
  • Board certification (AANP or ANCC)
  • Current BLS/CPR certification
  • MSN from an accredited nursing program
  • Minimum of 6 months NP experience preferred

Compensation Earn $80$90 per evaluation completed, with a mileage stipend and additional compensation for administrative time.
