Enjoy the autonomy and reward of conducting home health risk assessments as an experienced Nurse Practitioner. This per diem opportunity supports Medicare Advantage plan members in the Tullahoma area and Coffee County, TN, including Manchester and Beechgrove. Providers play a crucial role in improving patient outcomes by evaluating health needs and ensuring care continuityall while enjoying a flexible, self-driven schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Choose your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual home health risk assessments in patients homes
- Review and evaluate past and current medical history, active medical conditions, and medication lists
- Educate patients regarding chronic conditions and medication regimens
- Identify care gaps and make recommendations for intervention or follow-up
- No treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- MSN degree from an accredited nursing program
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- At least 6 months of recent Nurse Practitioner experience preferred
