Job Description
Make a meaningful impact as a Nurse Practitioner by delivering home health risk assessments to Medicare Advantage members in various locations throughout Coffee County, TN, including Tullahoma, Manchester, and Beechgrove. This per diem opportunity allows you to create a schedule that works for you, offering flexibility and autonomy in your practice.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule based on your availability
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Conduct home health risk assessmentseach taking approximately 45 minutesfor Medicare Advantage plan members
- Perform comprehensive evaluations of prior/current medical history, pre-existing conditions, and medication regimens
- Educate patients regarding their health conditions and medications
- Identify gaps in care and make recommendations for improved health outcomes
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Tennessee
- Board certification by AANP or ANCC
- Current CPR certification (BLS required)
- Graduation from an accredited MSN program
- At least 6 months of recent nurse practitioner experience preferred
