Make a meaningful impact in the community by performing comprehensive home health risk assessments for Medicare Advantage members throughout Jackson County, AR, including areas such as Newport, Tuckerman, and Diaz. This per diem opportunity allows you to work independently, supporting patients in their own homes and helping to ensure they receive high-quality, personalized healthcare.

Schedule:

• Flexible shifts available between Monday and Sunday
• Set your own schedule to fit your lifestyle
• Commitment can be minimum weekly or bi-weekly hourly blocks

Primary Responsibilities:

• Complete thorough annual risk assessments and post-discharge visits for Medicare Advantage plan members in their homes
• Review prior and current medical history, ongoing health conditions, and medication use
• Educate members on chronic conditions, preventive care, and appropriate medication management
• Identify and address care gaps by making recommendationsno direct treatment planning or prescribing is required

Required Qualifications:

• Active Nurse Practitioner license in Arkansas (AR)
• Current CPR certification (BLS)
• Board Certification (AANP or ANCC)
• MSN from an accredited nursing program
• At least 6 months of current Nurse Practitioner experience preferred

This role provides compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and remuneration for administrative tasks. Youll have the autonomy to create your own schedule, ensuring a work-life balance that supports your personal and professional needs.
