Take the lead in delivering home-based healthcare by supporting Medicare Advantage members in Jackson County, AR. This per diem opportunity is ideal for Nurse Practitioners seeking flexible shifts and autonomy, with the potential to make a meaningful impact through comprehensive home health risk assessments. Visits may range from annual evaluations to post-discharge check-ins, providing variety and direct patient engagement in locations such as Newport, Tuckerman, and Diaz.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Graduate of an accredited MSN program
- Board certification via AANP or ANCC
- Current CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience preferred
Key Responsibilities:
- Perform focused home health risk assessments for Medicare Advantage members
- Review and evaluate current/past medical and medication histories
- Educate patients on health conditions and medications to promote understanding and compliance
- Identify gaps in care and recommend practical solutions
- No prescribing of medications or development of treatment plans required
