Experienced Nurse Practitioners are needed to perform home health risk assessments within Jefferson County, AR. This per diem opportunity allows you to conduct comprehensive evaluations for Medicare Advantage members in Pine Bluff, White Hall, Redfield, and surrounding areas, providing valuable assessments in a home environment.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual health risk assessments, which average about 45 minutes per evaluation.
• Review and document prior and current medical history, chronic conditions, and medication usage.
• Educate patients on medical conditions and medication management.
• Identify gaps in care and offer recommendations to improve health outcomes.
• Visits may vary and include annual health assessments or post-discharge evaluations.
• No need to create treatment plans or prescribe medications.

Qualifications:

• Active Nurse Practitioner license in Arkansas.
• Current CPR certification (BLS required).
• National board certification through AANP or ANCC.
• Minimum of six months recent NP experience preferred.
• Masters degree (MSN) from an accredited nursing program.
