Advance your clinical skills by providing valuable care in the home health setting. As a Nurse Practitioner working per diem, youll perform in-home health risk assessments for Medicare Advantage plan members throughout Santa Cruz County, AZincluding Tubac, Rio Rico, and Sonoita. Each assessment averages 45 minutes and may involve annual health check-ups or post-discharge reviews, ensuring that patients receive comprehensive evaluations in a comfortable, familiar environment.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule based on your availability
- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform yearly or post-discharge in-home health risk assessments
- Review and evaluate medical history, current conditions, and medications
- Educate patients on disease processes and their medications
- Identify gaps in care and suggest actionable recommendations
- No prescribing medications or developing treatment plans
Qualifications:
- Valid Nurse Practitioner license in Arizona
- Current CPR/BLS certification
- Board Certified through AANP or ANCC
- Minimum 6 months of Nurse Practitioner experience preferred
- MSN from an accredited nursing school
