Job Description

Make a meaningful impact on patient care by bringing your expertise as a Nurse Practitioner to a per diem home health role focused on risk assessments. This opportunity involves traveling to members residences in Shelby County, Ohioincluding Sidney, Anna, Fort Loramie, and Russiato deliver care and conduct visits tailored to individual needs, such as annual health assessments and post-discharge evaluations for Medicare Advantage plan members.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule for ideal work-life balance
  • Commit to a minimum number of hours weekly or bi-weekly based on your availability

Key Responsibilities:

  • Perform annual home health risk assessments per Medicare Advantage protocols
  • Review and document prior/current medical history, health conditions, and medication use
  • Educate patients on managing their conditions and medications
  • Detect gaps in care and make recommendations for follow-up
  • Visits do not require prescribing medications or developing new treatment plans

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification via AANP or ANCC is required
  • CPR certification (BLS) must be current
  • Completion of MSN degree from an accredited institution
  • Prefer six months or more recent Nurse Practitioner experience

You will be compensated at $80$90 per evaluation, plus a mileage stipend and payment for administrative time.
