Advance your career in a dynamic per diem role designed for experienced Nurse Practitioners with a passion for home health. This opportunity focuses on conducting risk assessments for Medicare Advantage plan members throughout Miami County, including Troy, Piqua, Tipp City, Union, and surrounding areas. Enjoy the flexibility of creating your own schedule with shifts available any day of the week.
Schedule/Hours:
- Flexible shifts Monday through Sunday
- Self-scheduled; choose shifts that fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patients medical histories, current conditions, and medication use
- Provide education regarding health conditions and medications during each visit
- Identify gaps in care and document recommendations for improvement
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner License in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- MSN degree from an accredited institution
- At least six months of recent Nurse Practitioner experience preferred
This role offers compensation of $80$90 per completed evaluation, along with a mileage stipend and consideration for administrative time.
