Make a meaningful impact as a Nurse Practitioner in a per diem home health setting. This position involves conducting in-home risk assessments for Medicare Advantage plan members throughout Albany County, including communities such as Cohoes, Watervliet, and Westmere. Each assessment typically takes about 45 minutes and focuses on either annual health reviews or post-discharge visits. Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours and manage your caseload
• Select minimum weekly or bi-weekly hourly commitments based on your availability

Responsibilities:

• Perform comprehensive home health risk assessments tailored to Medicare Advantage members
• Review and evaluate prior/current medical history, ongoing conditions, and medication usage
• Offer patient-focused education related to conditions and prescribed medications
• Detect gaps in care and recommend actionable, relevant next steps
• This role does not require creating treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license in New York State
• Active BLS/CPR certification
• Board Certified (AANP or ANCC)
• Minimum of 6 months experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited program
