Nurse Practitioners passionate about home health assessments and preventive care have an exciting opportunity to make a meaningful impact. This per diem role focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members in Schenectady County, NY. Locations may include Schenectady, Rotterdam, and Glenville.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Ability to design and adjust your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate prior and current medical histories, health conditions, and medication regimens
- Provide education to members regarding their medical issues and prescribed medications
- Identify potential gaps in care and collaborate on recommendations for follow-up or support services
- Note: This position does not require creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New York State
- Board Certification through AANP or ANCC required
- Current CPR certification (BLS)
- Graduation from an accredited Masters in Nursing (MSN) program
- At least six months of recent Nurse Practitioner experience preferred
This position offers compensation of $80$90 per evaluation, with an additional stipend for mileage and administrative time.
