Schedule:
- Flexible shifts offered Monday through Sunday
- Set your own work hours to suit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for a Medicare Advantage health plan
- Review and evaluate participants medical and medication histories
- Educate patients on their underlying medical conditions and prescribed medications
- Identify gaps in care and offer informed recommendations for follow-up
- Note: This position does not require creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner License in NY
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- Preferred: At least 6 months of recent NP experience
- MSN from an accredited school of nursing
Clinical visits generally last around 45 minutes and may vary in nature, including annual health assessments or post-discharge visits. With pay ranging from $80$90 per evaluation, plus a mileage stipend and compensation for administrative time, this role offers both professional satisfaction and financial reward.
