Experienced Nurse Practitioners are invited to play a vital role conducting home health risk assessments for Medicare Advantage plan members throughout Rensselaer County. This per diem opportunity allows for true work-life balancechoose your own schedule and enjoy flexible shifts, Monday through Sunday, with the option for weekly or bi-weekly commitments.

Key Responsibilities:
- Travel within designated areas in Rensselaer County (including Troy, Rensselaer, and Hoosick Falls) to conduct comprehensive home-based risk assessments
- Complete annual and post-discharge health assessments, each averaging 45 minutes
- Review and evaluate patients current/past medical histories, conditions, and medications
- Provide health education tailored to each patients medical conditions and medication regimen
- Detect and document any care gaps; recommend actionable next steps to resolve them
- No treatment planning or medication prescribing necessaryfocus is strictly on assessment and education
Qualifications:
- Active Nurse Practitioner licensure in NY
- Board certified via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited program
- At least 6 months of recent Nurse Practitioner experience preferred
