A well-qualified nurse practitioner is sought to conduct home health risk assessments for Medicare Advantage plan members throughout Albany County, NYwith visits taking place in areas such as Cohoes, Watervliet, and Westmere. As a per diem provider, you will enjoy flexibility in your scheduling, working shifts that fit your availability. Evaluations typically last about 45 minutes and may include annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-directed schedulingcreate your own workweek
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Assess prior and current medical history, ongoing medical conditions, and medication usage
- Educate members on their health conditions, preventative care, and medications
- Identify care gaps and recommend follow-up or additional support as necessary
- No treatment planning or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in NY
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent nurse practitioner experience preferred
