Step into a vital per diem opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members across Albany County, NY. Your expertise will play a crucial part in comprehensive patient care, focusing on annual assessments and post-discharge visits. Each home visit is thoughtfully designed, averaging around 45 minutes, and may be scheduled in areas throughout Albany County, including Cohoes, Watervliet, and Westmere.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct detailed home health risk assessments for Medicare Advantage members
- Review past and present medical histories, current medical conditions, and medications
- Offer patient education relating to their conditions and prescribed medications
- Identify gaps in care and make recommendations to address them
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS required)
- Board Certification as an NP (AANP or ANCC)
- Minimum of 6 months recent NP experience preferred
- Masters degree in Nursing (MSN) from an accredited program
This position offers compensation of $80 to $90 per evaluation, along with a mileage stipend and paid administrative time.
