Take your advanced nursing skills into the community setting by performing comprehensive home health risk assessments. Utilizing your expertise as a Nurse Practitioner, you will conduct in-home evaluations for Medicare Advantage plan members throughout Randolph County, with potential visits in Asheboro, Archdale, and Trinity. Each home visit typically lasts about 45 minutes and may involve annual assessments or post-discharge evaluations, ensuring thorough and compassionate patient care right where it matters most.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment as needed
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review prior/current medical history, conditions, and medications
- Educate patients on their health status and medication regimen
- Identify gaps in care and suggest appropriate actions or interventions
- No requirements for treatment plan creation or medication prescribing
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent experience as a Nurse Practitioner preferred
