Schedule/Hours:

  • Flexible shifts between Monday and Sunday
  • Set your own workdays and hours
  • Commitment can be on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual and post-discharge health risk assessments for Medicare Advantage plan members residing in Gibson County (including Humboldt, Milan, and Trenton)
  • Review and evaluate prior and current medical histories, chronic conditions, and medication regimens
  • Educate patients about their conditions and medications to promote improved self-management
  • Identify care gaps and provide actionable recommendations to support member health
  • No duties related to prescribing medications or developing treatment plans

Required Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current BLS (CPR) certification
  • Board certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum of 6 months current NP experience (preferred)
