As a Nurse Practitioner focused on home health, you will conduct comprehensive risk assessments for Medicare Advantage members, ensuring high-quality care and meaningful patient interactions throughout Mercer County, New Jersey. In this role, youll travel to patients in various areas, including Trenton, Princeton, and Hamilton, and each visit will last approximately 45 minutes, offering variety and autonomy in your daily responsibilities.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling create your own work hours
• Minimal weekly or bi-weekly hourly commitment required

Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage health plan members
• Review medical history, evaluate current health conditions, and assess medication management
• Educate patients on their conditions and medications, fostering better health understanding
• Identify and document care gaps, making recommendations for resolution
• No development of treatment plans or medication prescribing necessary

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board Certified via AANP or ANCC
• MSN degree from an accredited nursing institution
• Current BLS/CPR certification required
• Minimum of 6 months experience as a Nurse Practitioner is preferred

Compensation ranges from $80$90 per evaluation, with an additional mileage stipend and paid administrative time.
