Job Description

Advance your career in a dynamic per diem role focused on Home Health risk assessments for Medicare Advantage plan members in Shawnee County, Kansas. As a Nurse Practitioner, you will conduct comprehensive assessments in members homes, spending approximately 45 minutes per visit. Assignments may include annual health assessments or post-discharge evaluations in locations such as Topeka, Silver Lake, and Auburn.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to customize your own schedule
  • Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage plan members
  • Gather and evaluate medical history, existing conditions, and medication usage
  • Educate patients on their medical conditions and medication regimens
  • Identify gaps in care, recommending solutions to enhance patient outcomes
  • No treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Kansas
  • Board Certification through AANP or ANCC
  • Masters degree in Nursing (MSN) from an accredited institution
  • Current CPR (BLS) certification
  • At least 6 months of recent experience as a Nurse Practitioner preferred
