Experienced nurse practitioners are needed to perform home health risk assessments for Medicare Advantage members throughout Kay County, OK, including the areas of Blackwell, Newkirk, and Tonkawa. This per diem opportunity offers maximum flexibility with self-crafted schedules and the option to work various shifts between Monday and Sunday. Each assessment, typically lasting approximately 45 minutes, may include an annual health evaluation or a post-discharge visit, depending on the patients needs.
Schedule:
- Flexible shifts available any day of the week, Monday to Sunday
- Design your own schedule
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments on Medicare Advantage health plan members
- Review and assess prior/current medical history, active medical conditions, and medication regimens
- Educate patients regarding their conditions and prescribed medications
- Identify care gaps and make recommendations to resolve them
- Documentation and assessment duties onlyno development of treatment plans or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in the state of Oklahoma
- Board Certification as an NP through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Active CPR (BLS) certification
- At least 6 months of nurse practitioner experience preferred
- Previous home health experience is an asset
