Step into a dynamic role focused on home health risk assessments for Medicare Advantage plan members across Niagara County, NY. This per diem Nurse Practitioner opportunity offers flexibility, autonomy, and the chance to positively impact patient care within the comfort of patients homes. Shifts are available throughout the week, allowing you to tailor your schedule to fit your needs, with the potential to work in locations including Niagara Falls, Lockport, and North Tonawanda.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete home health risk assessments averaging 45 minutes per patient
- Review prior and current medical history, ongoing conditions, and medication use
- Educate patients on medical issues, conditions, and medication management
- Pinpoint gaps in care and recommend solutions
- Focus solely on assessmentsno treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- Current Basic Life Support (CPR) certification
- Board certified through AANP or ANCC
- MSN from an accredited school of nursing
- At least 6 months of current NP experience preferred
This position offers compensation of $80-$90 per risk assessment, plus a mileage stipend and administrative time, rewarding your expertise and commitment to patient-centered care.
