Job Description
Home health Nurse Practitioners seeking flexibility and meaningful patient engagement will find a rewarding opportunity in this per diem role. This position involves conducting risk assessments for Medicare Advantage members in various locations throughout Ocean County, including Lakewood, Toms River, and Brick. Practitioners have the flexibility to create their own schedules, with shifts available any day between Monday and Sunday, and can opt for weekly or bi-weekly hourly commitments.

Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate patients medical histories, including current/past conditions and medication usage
- Educate patients on disease management and medication regimens
- Identify care gaps and offer practical recommendations for improved outcomes
- Focus exclusively on assessment and educationno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner License in New Jersey
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- At least 6 months of NP experience preferred
Nurse Practitioners will receive $80-$90 per evaluation, along with a mileage stipend and additional compensation for administrative time. This role offers a unique blend of professional autonomy and the chance to positively impact the well-being of Medicare Advantage members within their homes.
