Advance your Nurse Practitioner career by providing essential in-home healthcare services for Medicare Advantage plan members across Ocean County, NJ, including Toms River, Lakewood, and Brick. This per diem opportunity allows you to conduct comprehensive risk assessments and health education with a flexible, self-scheduled structure.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to suit your availability
- Options for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members
- Evaluate patients prior and current medical history, conditions, and medication regimens
- Deliver education regarding medical conditions and medications directly to members in their homes
- Identify gaps in care and make appropriate recommendations to enhance patient well-being
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Valid CPR certification (BLS)
- Board Certification through either AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience preferred
Enjoy competitive compensation of $80-$90 for each completed evaluation, with additional mileage stipend and administrative time allowance.
