Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments within James City County. Youll visit Medicare Advantage plan members residing across Williamsburg, Jamestown, and other areas within James City County, delivering vital assessments that help shape individualized healthcare strategies.

Schedule:

  • Flexible scheduling options with shifts available Monday through Sunday
  • Set your own hours and frequency
  • Minimum weekly or bi-weekly hour commitments

Key Responsibilities:

  • Conduct home health risk assessments for Medicare Advantage members, averaging 45 minutes per visit
  • Perform evaluations that may include annual health assessments or post-discharge follow-ups
  • Review prior and current medical histories, chronic conditions, and medications
  • Educate patients on disease management, preventive measures, and medication use
  • Identify gaps in care and recommend follow-up actions to enhance patient health outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Virginia
  • Current CPR certification (BLS)
  • Board-certified through AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience is preferred
