Step into a dynamic per diem opportunity as a Nurse Practitioner specializing in home health risk assessments within James City County. This position offers unmatched flexibility, allowing you to design your own schedule and maintain a healthy work-life balance. Youll visit Medicare Advantage plan members residing across Williamsburg, Jamestown, and other areas within James City County, delivering vital assessments that help shape individualized healthcare strategies.
Schedule:
- Flexible scheduling options with shifts available Monday through Sunday
- Set your own hours and frequency
- Minimum weekly or bi-weekly hour commitments
Key Responsibilities:
- Conduct home health risk assessments for Medicare Advantage members, averaging 45 minutes per visit
- Perform evaluations that may include annual health assessments or post-discharge follow-ups
- Review prior and current medical histories, chronic conditions, and medications
- Educate patients on disease management, preventive measures, and medication use
- Identify gaps in care and recommend follow-up actions to enhance patient health outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Virginia
- Current CPR certification (BLS)
- Board-certified through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience is preferred
