Job Description

Seeking a dedicated healthcare professional to provide home health services as a Nurse Practitioner. In this per diem position, you will conduct home health risk assessments for Medicare Advantage plan members across Howard County, serving communities such as Kokomo, Greentown, and Russiaville.

Schedule:

  • Flexible shifts, Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments, typically around 45 minutes per visit
  • Document evaluations of prior and current medical histories, review chronic and acute medical conditions, and assess medication management
  • Educate patients regarding their diagnoses and medications, supporting informed health decisions
  • Identify care gaps and offer actionable recommendations to promote optimal patient outcomes
  • Visits may include annual health check-ups or post-discharge assessments
  • No requirements to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months of recent Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution
