Job Description

Seeking a compassionate and detail-oriented Nurse Practitioner to conduct home health risk assessments on a per diem basis. This role offers variety and autonomy, as each assessmentusually lasting 45 minuteswill be performed for Medicare Advantage plan members across locations within Howard County, such as Kokomo, Greentown, and Russiaville. Youll be ensuring members receive personalized, informative care during annual assessments or post-discharge visits.

Schedule:

  • Flexible shifts available seven days per week
  • Design your own work schedule
  • Minimal weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments
  • Review prior and current medical history, chronic conditions, and medication use
  • Educate members about their health conditions and medication regimens
  • Detect potential care gaps and communicate recommendations for improvement
  • No responsibility for creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner (NP) license in Indiana
  • Valid CPR certification (BLS)
  • Board certified via AANP or ANCC
  • At least six months of current experience as an NP preferred
  • MSN degree from an accredited nursing program

Compensation for this position is $80-$90 per evaluation, with a mileage stipend and extra consideration for administrative work, making this an attractive opportunity for NPs seeking flexibility and rewarding patient interactions.
