Seeking an experienced Nurse Practitioner for per diem home health risk assessments within Desha County. In this flexible role, youll conduct in-person visits to Medicare Advantage plan members in communities such as Dumas, McGehee, and Arkansas City. Assessments average 45 minutes and may include annual wellness checks or post-discharge visits. You will coordinate schedules with a high level of autonomy and determine your own shift availability.
Schedule:
- Flexible shiftswork any day of the week, Monday through Sunday
- Choose your own schedule and commit to weekly or bi-weekly hours that fit your needs
Key Responsibilities:
- Complete thorough home health risk assessments for Medicare Advantage members
- Assess prior/current health history, chronic medical conditions, and medication regimens
- Educate patients regarding their health conditions and prescribed medications
- Recognize gaps in care and offer recommendations for improvement and follow-up
- No medication prescribing or development of treatment plans required
Qualifications:
- Active and unrestricted Nurse Practitioner license in Arkansas
- Board Certified through AANP or ANCC
- Master's degree in Nursing (MSN) from an accredited nursing program
- Current BLS (CPR) certification
- Minimum of 6 months recent Nurse Practitioner experience preferred (but not required)
This position offers compensation of $80$90 per completed evaluation, as well as a mileage stipend and additional pay for administrative time.
