Job Description

Deliver quality patient care in the home setting by providing comprehensive health assessments as a Nurse Practitioner with experience in Home Health. This per diem position offers a flexible schedule and the opportunity to impact Medicare Advantage plan members within Desha County. Your expertise will support annual health reviews and post-discharge visits, with each home health risk assessment typically lasting 45 minutes. Assignments may include residents in Dumas, McGehee, Arkansas City, and the surrounding area, with shift locations determined by patient needs.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling optionsdesign your own work week
  • Minimum hourly commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and evaluate current and previous medical history, existing conditions, and medication use
  • Provide patient education about health conditions and medication management
  • Identify care gaps and offer recommendations for addressing them
  • Documentation is required, but no treatment planning or medication prescribing

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing program
  • At least 6 months of recent NP experience preferred
