Advance your clinical expertise while making a meaningful difference in patients lives by providing home health risk assessments throughout Desha County, AR. As a Nurse Practitioner specializing in home health, you will perform comprehensive evaluations for Medicare Advantage plan members in their homes, ensuring continuity of care and supporting optimal patient outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hours commitment
Responsibilities:
- Conduct thorough annual home health risk assessments, including post-discharge visits, for plan members across Desha County (areas such as Dumas, McGehee, and Arkansas City)
- Review and evaluate prior and current medical histories, existing conditions, and medication regimens
- Offer education to patients about their medical conditions and prescribed medications, enhancing understanding and compliance
- Identify potential gaps in care and recommend practical solutions to address these issues
- Focus exclusively on assessment; prescribing medications or developing treatment plans is not required
Preferred Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current certification in CPR (BLS)
- Board Certification via AANP or ANCC
- Minimum of six months recent experience as a Nurse Practitioner in a home health setting
- MSN degree from an accredited nursing program
You will receive $80-$90 for each completed evaluation, plus a mileage stipend and administrative time compensation.
