Job Description

Advance your clinical expertise while making a meaningful difference in patients lives by providing home health risk assessments throughout Desha County, AR. As a Nurse Practitioner specializing in home health, you will perform comprehensive evaluations for Medicare Advantage plan members in their homes, ensuring continuity of care and supporting optimal patient outcomes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose a minimum weekly or bi-weekly hours commitment

Responsibilities:

  • Conduct thorough annual home health risk assessments, including post-discharge visits, for plan members across Desha County (areas such as Dumas, McGehee, and Arkansas City)
  • Review and evaluate prior and current medical histories, existing conditions, and medication regimens
  • Offer education to patients about their medical conditions and prescribed medications, enhancing understanding and compliance
  • Identify potential gaps in care and recommend practical solutions to address these issues
  • Focus exclusively on assessment; prescribing medications or developing treatment plans is not required

Preferred Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current certification in CPR (BLS)
  • Board Certification via AANP or ANCC
  • Minimum of six months recent experience as a Nurse Practitioner in a home health setting
  • MSN degree from an accredited nursing program

You will receive $80-$90 for each completed evaluation, plus a mileage stipend and administrative time compensation.
