Experienced Nurse Practitioners are sought for per diem home health roles conducting risk assessments for Medicare Advantage plan members in Albuquerque, Los Ranchos de Albuquerque, Tijeras, and other areas of Bernalillo County. This opportunity offers autonomy, flexibility, and the chance to positively impact patient outcomes through individual home visits.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Self-scheduling to fit your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual risk assessments in-home for Medicare Advantage members
• Assess patients medical and medication history, and document findings
• Educate patients about their conditions, medications, and healthy living strategies
• Identify potential gaps in care and make recommendations to close them
• Complete each assessment in approximately 45 minutes
• No prescribing or treatment plan creation required

Qualifications:

• Unrestricted Nurse Practitioner license in New Mexico
• Board Certification through AANP or ANCC
• Current CPR/BLS certification
• MSN from an accredited school of nursing
• Minimum of six months recent experience as a Nurse Practitioner preferred
