Opportunity available for a Nurse Practitioner to deliver vital home health risk assessments throughout Maury County, TN. This per diem position provides unmatched flexibility for professionals seeking autonomy and the chance to make a direct impact on patients lives. Assignments may be scheduled in Columbia, Spring Hill, Mount Pleasant, or other communities within Maury County.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
As a Nurse Practitioner in this home health setting, you will conduct comprehensive, approximately 45-minute risk assessments for Medicare Advantage health plan members. Each visit may involve an annual health assessment or a post-discharge evaluation, tailored to the patients recent healthcare events.
Responsibilities include:
- Completing thorough home health risk assessments
- Reviewing medical history, current conditions, and medication usage
- Educating patients about their health and medications
- Recognizing gaps in care and recommending appropriate follow-up
- No responsibility for treatment planning or prescribing medications
Ideal candidates will possess:
- Active Nurse Practitioner license in Tennessee
- Board certification from AANP or ANCC
- Minimum of six months NP experience preferred
- Master of Science in Nursing (MSN) from an accredited school
- Current CPR certification (BLS)
Compensation for this role is $80-$90 per completed evaluation, along with a mileage stipend and compensation for administrative tasks.
