Take the next step in your Nurse Practitioner career by providing essential home health services in Maury County, TN. This per diem role involves traveling to members residences throughout Columbia, Spring Hill, Mount Pleasant, and surrounding areas to conduct comprehensive home health risk assessments for Medicare Advantage health plan members.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to match your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual risk assessments and post-discharge visits in home settings
- Review and document prior/current medical history, medical conditions, and medication usage
- Educate members on their specific medical conditions and prescribed medications
- Identify any care gaps or health risks and provide solutions or recommendations
- No requirement for the creation of treatment plans or prescription of medications
Qualifications:
- Active Nurse Practitioner License in TN
- Current BLS/CPR certification
- Board Certified through either AANP or ANCC
- Minimum of 6 months experience as a NP preferred
- Master of Science in Nursing (MSN) from an accredited program
This position offers competitive rates of $80-$90 per completed evaluation. Mileage reimbursement and administrative time compensation are also included.
